Appointment Booking: Appointments can be booked in-person, over the phone, through our website or through Walk in. We require 3 hours prior notice for any cancellations or rescheduling. Failure to provide notice will result in a deduction of 50 percent of online deposited fee. 

Termination of Service: We reserve the right to terminate services for any reason, including but not limited to, patient non-compliance, disruptive behavior, or failure to pay for services. 

Medical Records: Medical records are confidential and will only be released with written consent from the patient or as required by law.​
